Transport

Slides must be securely packaged to prevent breakage or loss during transport. Specimens can be transported at room temperature in the range of 8 Degrees Celsius to 24 Degrees Celsius.

Registration

Please store samples at room temperature. Only place barcode labels on slide boxes; do not add barcode labels to individual slides. Please scan all referral paperwork and the send original referral with the sample.

Pre-testing Requirements

If you are a requesting clinician (e.g. oncologist), please contact the histology laboratory holding the FFPE block to arrange processing and submission of the specimen.

Additional Specimen Information

1. An H&E slide clearly marked by a histopathologist including % tumour content . A minimum of 75% tumour content within the targeted area is required.

2. A minimum of 10 unstained FFPE slides (10 micron sections)

3. A minimum of two patient identifiers must match between the request form and the slides provided, including the block number .

Test Method

The Illumina Infinium Methylation EPIC v2.0 assay is a microarray-based method of methylation profiling which covers thousands of CpG loci per sample (~930K). It combines bisulfite conversion of genomic DNA and PCR free whole genome amplification (WGA) with array-based capture and scoring of the CpG loci. Signal intensity is measured with the Illumina iScan in order to generate beta values of the relative degree of methylation at every CpG interrogated.
CNS tumour classifications can be obtained by uploading this raw DNA methylation data to the Heidelberg EPIGNOSTIX classifier, an online platform developed and prospectively tested at Deutsches Krebsforschungszentrum (DKFZ; German Cancer Research Center, Heidelberg, Germany) in a large series of paediatric and adult CNS tumours.
Some cases may be also uploaded to alternative classification algorithms such as the NCI/Bethesda classifier.

Limitations / Interference

Test results can be affected by factors such as intra-tumoral methylation heterogeneity, the presence of normal tissue contamination of the tumour sample (or tumour contamination on a normal tissue sample), PCR reaction inhibitors (mucin, necrotic material), or quality of the specimen. Therefore, the clinical diagnosis or therapy should not be solely based on this assay and the results should be considered in conjunction with clinical information and additional diagnostic tests.
