Tube must be filled up to the "Fill-Line". Factor VIII and IX may be required, depending on the APTT results. Please collect 2 citrate tubes. Label citrate tubes in order of draw. Write 1,2, etc in the space next to DOB

Give a photocopy of the request form to Coag dept., plus the excess barcode numbers in a biohazard bag
