Pre-testing Requirements

Labelling Requirements:
· Both container and the request form must be labelled with at least two patient identifiers (e.g., Full name, date of birth or NHI number).
· Site of specimen is also required.
· A requisition form must accompany the specimen container detailing relevant clinical details including any relevant previous Histology Pathology.

Additional Specimen Information

Specimen Transport
· Wrap tissue in saline-moistened gauze and place in a sealed container. Place the container into a biohazard bag and request form in the side pocket.
· Place and seal ice in another biohazard bag.
· Place this ice bag next to the specimen container in the first biohazard bag (the ice should NOT be in the container with the tissue).
· Use an APS priority transport bag, if unavailable, place in a brown paper bag and write on the front:
