If creatinine clearance is less than 30 ml/minute consider alternative agent or discuss with Infectious Diseases, Clinical Microbiology or Renal Services
IDEAL body weight:
Male: 50kg + 0.9 kg for every cm over 150cm in height
Female: 45.5kg + 0.9kg for every cm over 150 cm in height
Monitoring: measure TROUGH levels only:
24 hour regimen: Serum levels should be taken no earlier than 18 hours post dose
48 hour regimen: Serum levels should be taken no earlier than 36 hours post dose

Principle: Turbidimetric Inhibition Immunoassay
Reagents: Siemens Atellica CH Gentamicin
Analyser: Siemens Atellica Solution (NSH) and Atellica CI (WTH)
Limitations / Interference
Aminoglycosides structurally similar to gentamicin (e.g. netilmicin, sagamicin, and sisomicin) may significantly cross-react with this assay.
